Generate Questionnaire records based on Record permission field

I have an application records as below:

Tracking ID	Owner
1	Spoorthi
2	Spoorthi
3	Spoorthi
4	Jhansi
5	Jhansi

I need to generate only two records- 

i.e. one record for spoorthi and another for Jhansi

1. Is this possible? Can we create questionnaire records based on RP field in the questionnaire?

2. Can we map the names Jhansi and spoorthi to a RP field in the questionnaire?

	Link XYZ to the Assessment Questionnaire and create record via campaign
	Assessment ABC
